The creature magically

Beast that has a challenge rating no higher than 1/4, or back into its true form. It reverts to its true form if it dies. Any equipment it is wearing or carrying is absorbed or borne by the new form (the creature's choice).
Your form ripples, flesh twisting as bones shift unevenly. Limbs lengthen, but jagged seams remain. Skin mottles, flickering between shapes, edges blurring with imperfections. The transformation lingers, unstable, refusing to settle into a seamless disguise.
